Job Description Pride Health is seeking a compassionate and motivated Physical Therapist to join a Home Health team in Sarasota, FL 34238 . Job Description: In this role, the Physical Therapist will provide in-home rehabilitation services and work collaboratively with a multidisciplinary clinical team to help patients restore mobility, improve functional independence, and maintain their optimal level of physical functioning. Key Responsibilities Patient Evaluation & Treatment Provide physical therapy services to patients as prescribed by a physician in accordance with agency policies and federal/state regulations. Conduct initial and ongoing patient assessments , including OASIS assessments , to evaluate functional abilities and activities of daily living (ADLs). Develop, implement, and revise individualized plans of care based on patient needs and therapy goals. Care Coordination Collaborate with physicians and interdisciplinary healthcare teams to ensure effective patient care and treatment planning. Communicate patient progress, treatment responses, and any changes in condition to the physician and care team. Clinical Documentation & Compliance Maintain accurate documentation of patient evaluations, treatment plans, and outcomes. Ensure compliance with home health policies, clinical guidelines, and regulatory standards. Required Qualifications Education Doctor of Physical Therapy ( DPT ) from a CAPTE-accredited program . Experience 1–2 years of Physical Therapy experience required. Licensure & Certifications Active Florida Physical Therapist License . BLS Certification from the American Heart Association or American Red Cross. Skills Strong patient assessment and clinical decision-making abilities. Experience with OASIS assessments and care plan development . Excellent communication and care coordination skills. Preferred Qualifications At least 1 year of experience in home health physical therapy . Additional Information: Patient Care Setting: Home Health (in-home patient visits) Location: Sarasota, FL 34238 Job Type: Direct Hire (Full-time) Schedule: Full-time Day shift, approximately/ 40 hours per week Pay Range: $40 - $50/Hourly (Based on experience, skill level, and point-based visit system) Interested?
